Differentiated thyroid carcinoma (DTC) shows a relatively good prognosis compared with carcinomas of other organs and the 10-y survival rate of DTC is > 80%. However, metastases of DTC develop in 7 23% of patients; the distant metastases occur commonly in the lungs, bones, and brain, and the bones are the second common site of metastases of DTC. We found that the specificity and the overall accuracy of F-18 FDG PET for the detection of bone metastases in patients with DTC were higher than those of planar Tc-99m bone scintigraphy, and concluded that F-18 FDG PET is superior to planar Tc-99m bone scintigraphy because of its lower incidence of false-positive results in the detection of bone metastases of DTC (1). The sensitivity and accuracy of F-18 fluoride PET/CT for the detection of bone metastases of DTC are significantly higher than that of Tc-99m bone scintigraphy (planar). However, the sensitivity and accuracy of Tc-99m bone scintigraphy (planar) are improved near to those of F-18 fluoride PET/CT when SPECT is added to a planar scan. The sensitivity of F-18 FDG PET/CT is significantly lower than that of F-18 fluoride PET/CT or Tc-99m bone scintigraphy (SPECT) (2).
